Online Application For International Membership

Before you begin…

You must fall into at least one of the categories noted below to apply for international membership. Please select the most appropriate category and note the information and materials needed before beginning the online application.

1. Successful completion of an ACGME-approved or the Royal College of Physicians and Surgeons of Canada-approved vascular surgery training or equivalent vascular surgery training outside of the United States or Canada. If you fall into this category, please have the following information and materials available before beginning this application:

• If you are just completing training or have been in practice for one year or less, you will be asked to upload a letter of support (Word document or PDF) from your Program Director. Or, you may mail a hard copy. Mailing address will be noted on application form.
• If you are a trained vascular surgeon in practice for one or more years, you will be asked to upload letters of support (Word documents or PDFs) from two (2) SVS members. If you cannot obtain two (2)  letters of support from SVS members, the SVS Membership Committee may consider the submission of one letter from the international applicant’s home society. The international applicant must be a member of the recognized vascular society for his or her country. The home society letter would attest to membership in good standing in the society and would summarize the applicant’s qualifications for SVS membership. Or, you may mail two (2) letters of support. Mailing address will be noted on application form.

OR

2. Certification in Vascular Surgery from the United States, Canada, or equivalent. If you fall into this category, please have the following information and materials available before beginning this application:

• Letters of support. You will be asked to upload letters of support (Word documents or PDFs) from two (2) SVS members. This requirement can be waived in situations where Active or other International SVS members are not available to provide supporting letters. As an alternative, the SVS Membership Committee may consider substitution of a letter from the international applicant’s home society. The international applicant must be a member of the recognized vascular society for his or her country. The home society letter would attest to membership in good standing in the society and would summarize the applicant’s qualifications for SVS membership. Or, you may mail two (2) letters of support. Mailing address will be noted on application form.
• Certification numbers will be requested.

OR

3. Surgeons whose clinical practice is dedicated primarily to vascular surgery. If you fall into this category, please have the following information and materials available before beginning this application:

• Letters of of support. You will be asked to upload letters of support (Word documents of PDFs) from two (2) SVS members. This requirement can be waived in situations where Active or other International SVS members are not available to provide supporting letters. As an alternative, the SVS Membership Committee may consider substitution of a letter from the international applicant’s home society. The international applicant must be a member of the recognized vascular society for his or her country. The home society letter would attest to membership in good standing in the society and would summarize the applicant’s qualifications for SVS membership. Or, you may mail two (2) letters of support. Mailing address will be noted on application form.
• Details on hospital privileges.
• Details on medical school appointments.
• Licensure information.
• List of publications available to upload.
• List of presentations available to upload.
• List of research grants available to upload.
• List of cases available to upload.
