You must fall into at least one of the categories noted below to apply for active membership. Please select the most appropriate category and note the information and materials needed before beginning the online application.
1. Successful completion of an ACGME-approved or the Royal College of Physicians and Surgeons of Canada-approved vascular surgery training or their equivalent in the United States or Canada. If you fall into this category, please have the following information and materials available before beginning this application:
- Letters of Support. If you are just completing training or have been in practice for one year or less, you will be asked to upload a letter of support from your Program Director. If you are a trained vascular surgeon in practice for one or more years, you will be asked to upload letters of support from two (2) SVS members. When SVS members in the community are not available to provide letters of support, letters can be obtained from physicians in leadership positions at institutions where the applicant is a vascular surgeon. Please note: hardcopies of these letters may be mailed. Mailing address is noted on application form.
OR
2. Certification in Vascular Surgery from the United States or Canada. If you fall into this category, please have the following information and materials available before beginning this application:
- Letters of Support. You will be asked to upload letters of support from two (2) SVS members. When SVS members in the community are not available to provide letters of support, letters can be obtained from physicians in leadership positions at institutions where the applicant is a vascular surgeon. Please note: hardcopies of these letters may be mailed. Mailing address is noted on application form.
- Certification numbers will be requested.
OR
3. Surgeons whose clinical practice is dedicated primarily to vascular surgery. If you fall into this category, please have the following information and materials available before beginning this application:
- Letters of Support. You will be asked to upload letters of support from two (2) SVS members. When SVS members in the community are not available to provide letters of support, letters can be obtained from physicians in leadership positions at institutions where the applicant is a vascular surgeon. Please note: hardcopies of these letters may be mailed. Mailing address is noted on application form.

- Licensure information.
- List of publications available to upload.
- List of presentations available to upload.
- List of research grants available to upload.
