Speakers and Moderators must download, complete and return the appropriate forms:
1. Conflict of Interest Disclosure Form Download the mandatory disclosure form. In accordance with ACCME guidelines, all moderators and speakers must disclose possible conflicts of interest prior to the presentation.

Vascular Annual Meeting® PowerPoint Presentation and Disclosure Template
Download the 2013 Vascular Annual Meeting PowerPoint Template to use in your presentation. When prompted to "open" or "save" this file, select "save" in order to manipulate this template.
SVS requests all SVS speakers to include a disclosure slide at the beginning of their presentations, including the following information: (a) name of the speaker; (b) any conflicts of interest with the name of the commercial interest(s); and (c) nature of the relationship the speaker has with each commercial interest. Even if a speaker has nothing to disclose, this slide must be included indicating "Nothing to disclose."
Meeting Registration and Manuscript Submission
SVS does not reimburse expenses or cover registration fees for speakers who are members of SVS.
Manuscript submission to the Journal of Vascular Surgery® is mandatory for those accepted to present at the Plenary and Peripheral Vascular Surgery Society (PVSS) Sessions of the 2013 Vascular Annual Meeting. Manuscripts should be submitted electronically to the Journal of Vascular Surgery at www.editorialmanager.com/jvs no later than June 10, 2013.
