Reseach Opportunity Submission Form Instructions

Note: do not use your browser's "Back" button before submitting the research opportunity. You will lose your information

 

(1) Note that you CANNOT use the following 5 characters in any field

	'	apostrophe
	"	quote
	<	less than
	>	greater than
	&	ampersand
	If you enter one of these characters in error, when you try to go to the next field, you will receive an error message saying "please remove erroneous character".  After you clear the error message, you can delete the erroneous character.

(2) For "Research Opportunity" submissions, the following fields are available. Those in bold are required fields and MUST BE COMPLETED.

	Research Opportunity Name	provide a title
	This Research Opportunity is for a...	Medical Student, Resident or Either
	Description [Maximum of 1500 characters]	Fill out a detailed description of the Research Opportunity
	Institution Name
	Institution Department
	Location -City, State/Province and Country
	Other, or Related Information [e.g WebSite URL] [Maximum of 200 characters]
	Contact Email
	Contact Information [Name, address, telephone and fax]	Submit a Contact Name, Address, Telephone and Fax Number
	Expiration Date
