Welcome to the second edition of On the Cutting Edge, an e-newsletter from the Society for Vascular Surgery reporting new training programs, up-to-date vascular research and other vascular-related information for residents and
students.

Six New Vascular Surgery Training Programs Available

In 2006 the ACGME approved new training paradigms that lead to certification in vascular surgery only. The new paradigms shorten the overall duration of training and provide more concentrated time to include the evolving new techniques within vascular surgery.

The first is a five-year integrated program of which three years are devoted to vascular surgery and two years to core surgical training. The second is a six-year program that consists of three years vascular surgery training following three initial years of preliminary general surgery training in the same institution. Both of these new programs are selected by fourth-year medical students in the standard matching program. In addition to these new possibilities, trainees may continue to pursue both general and vascular
surgery training in the current sequential programs. Please visit the ACGME web site at:

New Brain Lesions Appear on Magnetic Resonance Imaging after Carotid Artery Stenting

Researchers from San Francisco, have recently published a study regarding patients who had brain
imaging (diffusion weighted MRI or DWI) before and after carotid artery stenting (CAS), to determine if there
were new lesions in patient's brains following the procedure.

Angioplasty and stenting fractures the atherosclerotic plaque as it pushes the plaque out of the way. To prevent plaque fragments from traveling to the brain protective distal filtration devices are used during CAS. However, some fragments do reach the brain and although strokes after these procedures are unusual, imaging using DWI does show small areas of damage.

Researchers asked if these fragments can cause damaged areas and if imaging early after the procedure would underestimate the number of these "silent" lesions after CAS. The findings were published in the May
2007 issue of the Journal of Vascular Surgery.

Aging Anatomic Factors Affects Carotid Angioplasty and Stenting

Carotid stenting can be performed on any age patient, but should be undertaken with caution and skill for
patients 80 years and older, as well as done safely in experienced centers, according to a study in the May
2007 issue of the Journal of Vascular Surgery.

The study suggests that patients older than 80 are more likely to have vascular changes that occur with age, predisposing them to increased risks for complications, including strokes.
